Hospitals, clinics, care teams, case managers, health plans, employers, and other organizations can use this form to accept MedicalRide.org's directory and request-platform terms.

Platform role

MedicalRide.org is operated by Tipmunk SASU, a French société par actions simplifiée unipersonnelle (SASU), share capital €1, registered with the Paris Trade and Companies Register under number 107 697 385, with registered office at 47 rue Vivienne, 75002 Paris, France. MedicalRide.org is the public-facing platform name used for directory listings, structured transportation requests, provider outreach, quote comparison, and related support workflows.

MedicalRide.org is a directory and request platform. It is not the transportation provider, motor carrier, ambulance operator, medical escort provider, health plan, insurer, or medical provider. Independent providers are responsible for determining whether they can lawfully and safely perform a requested trip.

Organization use

The organization may use MedicalRide.org to find listed providers, submit structured transportation requests, compare provider responses, and coordinate with independent providers. The organization remains responsible for its own authorization, patient/member relationship, notices, consents, minimum-necessary decisions, and records.

Do not submit clinical records, Social Security numbers, full payment card numbers, or information unrelated to transportation coordination. Privacy details are available in the Privacy Policy.

Intended use

I am authorized to submit this agreement for the organization named above.I agree that MedicalRide.org is a directory and request platform operated by Tipmunk SASU. It is not the transportation provider, carrier, ambulance company, insurer, health plan, or medical provider.I understand independent providers are responsible for services, drivers, vehicles, licensing, insurance, pricing, communications, and ride completion.The organization will submit only information reasonably needed for directory/request coordination and will not send clinical records, Social Security numbers, or full payment card numbers through this form.I agree that typing my name and submitting this form creates an electronic acceptance record for this organization agreement.
